Below are the documents and forms common to our life and ministry in the Northwest Region. Some of these files can be found in other locations on the website, others are specifically found here only. Please don't hesitate to alert the Regional Office if you discover a discrepancy.
Some forms may be filled out online. Those options are noted. All other documents and forms are in PDF format and available for you to print.

| Expense Reimbursement & Donation Forms |

| Have you recently incurred expenses for business you've done on behalf of the Region? Would you like to document expenses you incurred while volunteering for the Region and recieve a tax record receipt? Please use one of these forms. |
1. Expense & Reimbursement Statement - PDF
Please print and complete the form. For reimbursements you must include receipts. Scanning and emailing is acceptable, as is a fax. If you would like to make your expenses a donation, you may fill out the printed form (and check the bottom box) or the online form (below) and a donation receipt will be provided to you.

| Outdoor Ministry Counselor/Adult Voluneer and CIT Application Form |
|
| Per the NWRCC Child Safety Policy, all volunteers who will work with children at a Region-sponsored event must have a completed Outdoor Ministry Counselor Application Form on file. This form is resubmitted in full every three years and a shorter renewal sheet is to be completed each year. These forms provide the Region with up-to-date references and permission to perform background checks. |

2b. Counselor/Adult Volunteer Yearly Renewal - Online Form
Complete the online form if you are over 18 and you've got a valid application on file, complete the PDF form if you are under 18 or prefer to mail or fax in your information. The form alerts the Region that you plan to be a counselor or CIT at an event and gives permission to perform the background check and update medical history.

| Regional Youth Council Application Form |
|
The Regional Youth Commission of the Northwest Regional Christian Church (Disciples of Christ) is a body of
young people, serving on the RYC during while in grades 10, 11 and 12 along with their Adult Sponsors. You
must currently be in at least Grade 9 to apply.

| Clergy Misconduct Policy and Procedure |
"The Northwest Regional Christian Church (Disciples of Christ) has affirmed through our
mission statement that we are called "to do justice, love kindness, and walk humbly with
God." (Micah 6:8). We believe that this scripture frames an appropriate response for
dealing with accusations of sexual misconduct and other ethical violations among clergy
with standing."
